What you share
When you request a consultation, the form asks for your name and email address, and optionally your phone number, due date or timing, city or region, expected birth setting, the support you're interested in, how you heard about Motherhood Multiplied, and a message. If you marked any birth preferences on the homepage, those choices are included too.
Why it's used
Only to reply to your inquiry and arrange a consultation. Your details are never sold, and you won't be added to a mailing list without asking.
Where it goes
Form submissions are collected by the website's hosting provider and delivered to Amanda at Motherhood Multiplied. The preferences you mark on the homepage stay in your own browser tab until you close it, and only leave it if you send the form.
Cookies and tracking
This website does not use advertising cookies, analytics, or tracking scripts.
Health information
Please share only what you're comfortable sharing. Doula support is nonmedical, and this form is not monitored for urgent needs. If you have a medical concern or emergency, contact your care provider or call 911.
Your choices
You can ask for the details you've shared to be corrected or deleted at any time by replying to any message from Motherhood Multiplied.
